Enrollment
We accept children (ages 2 thru 6) and adults (age 22 and above) with various developmental disabilities. There will be no discrimination on basis of race, color, religion, gender, national origin or disability. Our center shall not admit or maintain any child whose needs it obviously cannot meet or whose behavior would be dangerous for other children. (DPW Reg. 470IAC 3-4, 1-7)
Admission will be based upon:
1. Children ages 2-5 with a mild, moderate or severe developmental disability.
2. Children ages 5-6 with a severe disability.
3. Adults ages 22 and above with a moderate or severe developmental disability.
4. Physical must have occurred within the twelve (12) month period prior to a new enrollment.
5. Parent’s signature on all necessary forms
6. For children- A physical order for physical/occupational/speech therapy as deemed necessary by child’ physician. A copy of a current immunization record and physical signed by a physician.
